Please find buttons to click on for the validated questionnaires my practice uses in order to initially assess the degree of pain and disability related to a joint problem and we use the same forms to compare a “before” and “after” surgery result in terms of long-term outcomes.  My staff will on a fairly regular basis contact you to complete these forms.  This is vital information and I realise it takes up some of your time but in the long run will facilitate scientific re-evaluation of procedures in order to continually improve our service.  These results will ultimately be used in academic presentations for the greater good with respect to medical care and education.
